Exosomes are biologically significant structures that have attracted considerable scientific interest. However, the term “exosome” does not refer to a standardized treatment that can be used for every disease, has the same characteristics in every product, or can guarantee healing on its own.

What Is an Exosome?
Exosomes are one type of very small extracellular vesicle released by cells into their surroundings. They are enclosed by a lipid membrane similar to the cell membrane. They may contain proteins, lipids, and certain molecules that carry genetic information.
These small structures are considered part of the communication system between cells. In simple terms, exosomes can be thought of as biological message packages sent from one cell to another.

No. An exosome is not a stem cell.

Exosomes are not ordinary serums that can be assumed to be risk-free
For biological products intended for use in the human body, the source, manufacturing conditions, microbiological safety, quality control, and traceability are critically important. A product is not automatically safe simply because it is natural or biologically derived.
The United States Food and Drug Administration (FDA) has issued a public safety notification regarding the marketing of unapproved exosome products with unsupported claims that they can prevent, treat, or cure diseases. Why Are Exosomes Being Studied Scientifically?

The first question should be:
Why is this wound not healing?
A chronic wound may have several underlying causes:
- Insufficient arterial blood flow
- Venous insufficiency and increased venous pressure
- Significant swelling of the leg
- Diabetes-related nerve damage
- Infection
- Continuous pressure on the foot or a specific area
- Inadequate wound care
- Malnutrition
- The simultaneous presence of more than one contributing factor
Particularly in leg and foot wounds, it is not sufficient to examine only the surface of the wound. Blood flow, edema, signs of infection, and the underlying cause of the wound must be evaluated together.

These wounds are called venous ulcers.

Over time, diabetes can affect both the nerves and the blood vessels. When nerve damage develops, a patient may not notice a small cut, blister, callus, or irritation caused by footwear.

- What established and evidence-based treatments are available for this purpose?
- What are the alternative approaches?
- What are the potential risks and uncertainties?
- Does the procedure comply with the relevant regulations and authorization requirements?
A scientific approach requires an open discussion not only of potential benefits, but also of limitations, risks, and alternatives.
